Insurance and Payment

*Most insurances are accepted. Benefits will be verified prior to your appointment. Please download and complete these forms and bring them to your first appointment, along with your driver's license and insurance card. All co-payments are due at the time of service.  I accept cash, check, HSA cards and all major credit cards. Please note that if you use a credit card a 4% processing fee will be added. Thank you.
